Connecticut Labradoodles_Deposit and Application Form

This application form helps me to reserve the best puppy suited for your family. Reservations are accepted in order of receipt of this form, and a non-refundable deposit of $300.00 will be required when your application is approved. Your deposit will be applied to the total purchase price (2,500) of the puppy. This price includes spaying or neutering of your puppy. CT residents add 6% sales tax.
