This application form helps me to reserve the best puppy suited for your family. Reservations are accepted in order of receipt of this form, and a non-refundable deposit of $300.00 will be required when your application is approved. Your deposit will be applied to the total purchase price (2,500) of the puppy. Connecticut residents please add a 6.35% sales tax.
